CDHCI is an Alberta Health Services program that lets families choose their own home care provider for covered personal care, homemaking, and respite caregiver support. 

With CDHCI, your care is funded, but your family stays in charge.

Who Qualifies?

  • Alberta residents with a valid health card

  • Care needs that can be safely met at home

  • Eligibility assessed and approved by an Alberta Health Services case manager

How to Get Started

  1. Guidance Through Approval – We provide contact numbers and help you prepare for your assessment with an AHS case worker.

  2. In-Home Nursing Assessment – Once approved, our nurse founder visits your home to assess needs and create a personalized plan.

  3. Alberta Blue Cross Setup – You will recieve a letter from Blue Cross with your file number. When you provide us with a copy, we can get started. We manage the paperwork and billing so you can focus on your family.

  4. Caregiver Matching – We introduce 2–3 carefully selected caregivers and then ask you to choose who feels right.

  5. Care Begins – Ongoing updates, nurse monitoring, and a care team that stays.

How do I know if I qualify for CDHCI?

If you're an Alberta resident with a health card and your care needs can be safely met at home, you likely qualify. An AHS Case Manager makes the final determination. We can help you start the process.

How long does CDHCI approval take?

Typically 2–4 weeks. We help you prepare for a smooth start to care while you wait.

Can I choose my caregiver?

Yes. You'll meet several caregivers and choose who feels like the best fit for your family.

What if my needs change?

Our RN case manager regularly reviews and updates your care plan.
